Sports Physicals for SY 2018-19

As most of you have heard Monroe Clinic no longer offers athletic/sport physicals, for $20 here at DHS over the summer.  I have included and attached information from Monroe Clinic that will give you an option to receive a "Wellness Exam" which would cover IHSA requirements to play sports next school year.  If you have any questions please contact Monroe Clinic at the number listed in the information below.

Thank you and have a great summer!
Chad Ferguson
Dakota Activities Director

"Hello,

Last year, Monroe Clinic began an initiative to encourage adolescents to have a full wellness exam instead of a sports physical. The purpose of this initiative is to make sure kids are up to date on immunizations, have had appropriate discussions with their physician around prescription needs, preventative and adolescent healthcare issues, depression screening and discussion of social interactions that impact health and attitudes. We have extended our hours for easier accessibility to receive a wellness exam.  This initiative aligns with the recommendations of the American Academy of Pediatrics therefore, this year we will not offer set dates for PPE’s.

Please feel free to communicate this change via email or school communication with your coaches, parents and athletes and please include the attached informative brochure and poster on the benefits of a Wellness Exam. Feel free to print posters and brochures as needed.  Your athletes can make a wellness appointment with their own pediatrician or with one of the Monroe Clinic locations most convenient for them. For more information about location and phone numbers for Wellness visits, please visit www.monroeclinic.org/sportsmedicine or call Sports Medicine at 608-325-PLAY (7529)."

Thank you again for your help with this initiative.
